COLLIN COUNTY HEALTH CARE FOUNDATION
BOARD OF TRUSTEES
Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
1. Call to order. The board will convene in open session for consideration of the following business matters:
2. Consent agenda to approve:
a. Disbursements for the period ending May 22, 2018, Auditor.
b. Amendment to the contact with the Texas Department of State Health Services for Contract No. 537-18-0128-00001, Amendment No. 01: Public Health Emergency Preparedness (PHEP) Hazards Grant to accept the FY 2019 grant award and further extend the contract through and including June 30, 2019, Health Care.
c. Amendment to the contact with the Texas Department of State Health Services for Contract No. 537-18-0348-00001, Amendment No. 01: Public Health Zika Response Activities Grant to extend the contract through and including July 31, 2019, Health Care.
d. Receive and File, Monthly Financial Reports for March 2018, Auditor.
e. Filing of the Minute(s), County Clerk:
1. May 7, 2018.
2. May 14, 2018.
EXECUTIVE SESSION
Executive Session, in accordance with Chapter 551 of the Government Code.
The board reserves the right to convene into executive session throughout this meeting.
