Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
HEALTH CARE FOUNDATION MEETING
1. Call to order. The board will convene in open session for consideration of the following business matters:

a. Disbursements for the period ending March 17, 2020, Auditor.
b. Agreement(s):
1. Services Agreement and Business Associate Contract with Allen Community Outreach for sick medical visits and prescription assistance effective March 1, 2020 through and including February 28, 2021, Health Care.
2. Services Agreement and Business Associate Contract with Community Health Clinic for sick medical visits and prescription assistance effective March 1, 2020 through and including February 28, 2021, Health Care.
3. Services Agreement and Business Associate Contract with Community Health Service Agency, Inc. for sick medical visits and prescription assistance effective March 1, 2020 through and including February 28, 2021, Health Care.
4. Services Agreement and Business Associate Contract with Hope Clinic of McKinney for sick medical visits and prescription assistance effective March 1, 2020 through and including February 28, 2021, Health Care.
5. Services Agreement and Business Associate Contract with The Samaritan Inn for sick medical visits and prescription assistance effective March 1, 2020 through and including February 28, 2021, Health Care.

1. Compliance Audit Report - Health Care (3rd and 4th Quarter FY2019), Auditor.
1. Acceptance of the FY 2021 Cities Readiness Initiative Grant (Contract No. 537-18-0141-00001) with the Texas Department of State Health Services and further finalize and execute Amendment No. 4 for same, Health Care.
2. Acceptance of the FY 2021 Public Health Emergency Preparedness Grant (Contract No. 537-18-0128-00001) with the Texas Department of State Health Services and further finalize and execute Amendment No. 4 for same, Health Care.

The board reserves the right to convene into executive session throughout this meeting.
