Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
HEALTH CARE FOUNDATION MEETING
1. Call to order. The board will convene in open session for consideration of the following business matters:
2. Consent agenda to approve:
a. Disbursements for the period ending May 14, 2019, Auditor.
b. Amendment to the contract with the Texas Department of State Health Services for Contract No. 537-18-0128-00001, Amendment No. 02: Public Health Emergency Preparedness (PHEP) Hazards Grant to accept the FY 2020 grant award and further extend the contract through and including June 30, 2020, Health Care.
c. Amendment to the contract with the Texas Department of State Health Services for Contract No. 537-18-0141-00001, Amendment No. 02: Cities Readiness Initiative Grant to accept the FY 2020 grant award and further extend the contract through and including June 30, 2020, Health Care.
d. Amendment to the contract with the Texas Department of State Health Services for Contract No. HHS000119700018, Amendment No. 01 to Immunization/Locals Grant Program to accept the FY 2020 grant award and further extend the contract through and including August 31, 2020, Health Care.
e. Personnel Changes, Human Resources.

The board reserves the right to convene into executive session throughout this meeting.
