FYI NOTIFICATION

1. Notification of budget adjustment(s)/amendment(s), Auditor:

a.AI-34763 $68,931 to establish the budget for the FY2012 Risk Based Funding (RBF) Grant.

b.AI-34774 $135,989 to establish the budget for the FY2012 Department of State Health Services (DSHS) Tuberculosis (TB) Federal Grant.

b.AI-34772 Memorandum of Understanding with the Collin County Community College District to utilize the District's available facilities and staff to assist in the event of a public health emergency and further authorize the Board President to finalize and execute same, Homeland Security.

c. Services Agreement(s) and Business Associate Contract(s) (in accordance with HIPAA and Incorporated Security Addendum) to provide assistance for low income United States Citizens and Resident Aliens of Collin County at or below 100% of the Federal Poverty Level through and including December 31, 2012 and further authorize the Board President to finalize and execute same, Health Care:
