HEALTH CARE FOUNDATION MEETING

1. Call to order. The board will convene in open session for consideration of the following business matters:

a.AI-34294 Disbursements for the period ending September 20, 2011, Auditor.

b.AI-34217 Services Agreement and Business Associate Contract with PrimaCare to provide primary and preventive health care to low income United States citizens and legal residents of Collin County through and including September 30, 2012 and further authorize the Board President to finalize and execute same, Health Care.

3. AI-34243 Designation of the FY2012 Health Care Foundation regular term meeting dates and time, Health Care.
