NOTICE OF A REGULAR MEETING OF THE
COLLIN COUNTY HEALTH CARE FOUNDATION
BOARD OF TRUSTEES
Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
1. Call to order. The board will convene in open session for consideration of the following business matters:
2. Consent agenda to approve:
a. Disbursements for the period ending September 8, 2020, Auditor.
b. Amendment(s):
1. No. 3 to Professional Services Agreement and Business Associate Agreement (Agreement No. 2017-249) with Jerry Barnett, R. Ph. to extend the contract for one (1) year though and including August 31, 2021, add Force Majeure language, and further authorize the Purchasing Agent to finalize and execute same, Health Care.
2. No. 5 to Lease Agreement with Collin County Health Care Trust Foundation and Collin County for property located at 900 E. Park Boulevard, Suite 140A-C & Suite 210, Plano, Texas to extend the lease for five (5) years through and including September 30, 2025, Construction & Projects.
3. No. 7 to the Lease Agreement with Collin County Health Care Trust Foundation and Collin County for property located at 900 East Park Boulevard, Suite 200, Plano, Texas to extend the lease for five (5) years through and including September 30, 2025, Construction & Projects.
4. No. 7 to the Lease Agreement with Collin County Health Care Trust Foundation and Collin County for property located at 920 E. Park Boulevard, Plano to extend the lease for five (5) years through and including September 30, 2025, Construction & Projects.
c. Receive and File, Auditor:
1. Monthly financial reports for July 2020.
d. Filing of the Minute(s), County Clerk:
1. August 10, 2020.
e. Miscellaneous
1. Acceptance of the FY2021 Women, Infant, and Children (WIC) grant contract (Contract No. HHS000801700001) with the Department of State Health Services, Health Care.
2. Personnel Appointments, Human Resources.
3. Personnel Changes, Human Resources.
EXECUTIVE SESSION
Executive Session, in accordance with Chapter 551 of the Government Code.
The board reserves the right to convene into executive session throughout this meeting.
