NOTICE OF A REGULAR MEETING OF THE
COLLIN COUNTY HEALTH CARE FOUNDATION
BOARD OF TRUSTEES
Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
1. Call to order. The board will convene in open session for consideration of the following business matters:
2. Consent agenda to approve:
a. Disbursements for the period ending March 27, 2018, Auditor.
b. Agreement(s):
1. Services Agreement and Business Associate Contract (in accordance with HIPAA and Incorporated Security Addendum) (NTE $35,025) with Allen Community Outreach to provide assistance for low income United States Citizens and Resident Aliens of Collin County at or below 100% of the Federal Poverty Level through and including February 28, 2019, Health Care.
2. Services Agreement and Business Associate Contract (in accordance with HIPAA and Incorporated Security Addendum) (NTE $40,000) with The Samaritan Inn to provide assistance for low income United States Citizens and Resident Aliens of Collin County at or below 100% of the Federal Poverty Level through and including February 28, 2019, Health Care.
c. Amendment No. 1 to the agreement with Oxford Immunotec, Inc. (Contract No. 2017-277) to reduce the cost for the Tuberculosis Lab Testing (T-Spot) from $45 to $36 per test, Health Care.
d. Receive and File, Monthly Financial Reports for November 2017, Auditor.
e. Filing of the March 12, 2018, Minutes, County Clerk.
f. Miscellaneous
1. InterLocal Application for Immunization Program Funds for FY 2019 through the Texas Department of State Health Services (DSHS), Health Care.
2. Inter-Local Application for the Tuberculosis Prevention and Control for FY 2019 State Funds through the Texas Department of State Health Services (DSHS), Health Care.
3. Grant application for the FY 2019 Cities Readiness Initiative (CRI) Grant through the Texas Department of State Health Services (DSHS), Health Care.
4. Grant application for the FY 2019 Hazards/Public Health Emergency Preparedness (PHEP) Grant through the Texas Department of State Health Services (DSHS), Health Care.
EXECUTIVE SESSION
Executive Session, in accordance with Chapter 551 of the Government Code.
The board reserves the right to convene into executive session throughout this meeting.
