NOTICE OF A REGULAR MEETING OF THE
COLLIN COUNTY HEALTH CARE FOUNDATION
BOARD OF TRUSTEES
Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
1. Call to order. The board will convene in open session for consideration of the following business matters:
2. Consent agenda to approve:
a. Disbursements for the period ending December 3, 2019, Auditor.
b. Acceptance of the FY2020 Tuberculosis Prevention and Control Grant (Contract No. HHS000686100011) with the Department of State Health Services effective January 1, 2020 through and including December 31, 2020, Health Care.
c. Amendment(s):
1. No. 5 to the Lease Agreement with Collin County WIC Program for property located at 825 N. McDonald Street, McKinney, Texas to extend the lease for a five (5) year period effective January 1, 2020 through and including December 31, 2024, and decrease lease floor area 653 square feet, Construction & Projects.
2. Amendment and application to the FY2020 TB Prevention and Control Grant through the Department of State Health Services to increase funding and establish funding for the FY2021 grant, Health Care.
d. Filing of the Minute(s), County Clerk:
1. November 11, 2019.
2. November 18, 2019.
EXECUTIVE SESSION

The board reserves the right to convene into executive session throughout this meeting.
