Notice is hereby given that the Collin County Health Care Foundation Board of Trustees will meet in REGULAR SESSION. Following is the agenda for said meeting:
HEALTH CARE FOUNDATION MEETING
Call to order. The board will convene in open session for consideration of the following

a. Al-39107 Disbursements for the period ending November 4, 2014, Auditor.
b. Al-3911 0 Filing of the October 20, 2014, Minutes, County Clerk.
c. Al-39118 Acceptance of the FY2015 Public Health Emergency Preparedness (PHEP) Supplemental Grant in the amount of $34,320; Contract No. 2015-047207 with the Texas Department of State Health Services (DSHS) beginning October 1, 2014 through and including June 30, 2015 and further authorize the Board President to finalize and execute same, Homeland Security.
d. Al-39095 Termination of the Lease Agreement with the Collin County Health Care Trust Foundation and Collin County Homeland Security for property located at 920 East Park Boulevard, Plano, Texas, Homeland Security.
