Health Services (DSHS) for Contract No. 2014-001394, Amendment No.
01: Tuberculosis Prevention and Controlâ?"State to make various
changes to the contract with a retroactive effective date of
September 1, 2013 and further authorize the Board President to
finalize and execute same, Health Care.
c.38115 Contract amendment with the Texas Department of State
Health Services (DSHS) for Contract No. 2014-001388, Amendment No.
02: Tuberculosis Prevention and Controlâ?"Federal to add a
non-recurring Progress Report for activities during January 1, 2014
to May 31, 2014, due July 15, 2014 with a retroactive effective date
of September 1, 2013 and further authorize the Board President to
finalize and execute same, Health Care.
