No. 1 to Storage and Microfilm Creation (RFP No. 2016-153) with Iron Mountain Information Management, LLC to make various changes to the contract, extend the contract for One (1) year through and including September 30, 2019 and further authorize the Purchasing Agent to finalize and execute same, County Clerk.

2. No. 5 to Frontier Parkway Improvements (AGR No. 2016-229) with Birkhoff, Hendricks & Carter, L.L.P. to address access through the Prosper ISD property and bid Prairie Crossing as a separate project and further authorize the Purchasing Agent to finalize and execute same, Engineering.

3. No. 8 to Voter Registration and Early Voting Software (Contract No. 02376-09) with VOTEC Corporation to make various changes to the contract, extend the contract for one (1) year through and including September 30, 2019 and further authorize the Purchasing Agent to finalize and execute same, Information Technology.

h. Change Order(s):

1. No. 1 to Printing; General Office Forms (IFB No. 2016-125) with Filex Systems, Inc. to include a price redetermination, extend the contract for one (1) year through and including September 30, 2019 and further authorize the Purchasing Agent to finalize and execute same, Purchasing.

2. No. 3 to Supplies: Veterinary and Animal Care (IFB No. 2017-074) with C. Specialties, Inc. to include a price redetermination, extend the contract for one (1) year through and including September 30, 2019 and further authorize the Purchasing Agent to finalize and execute same, Development Services.

1. Acknowledgment of the issuance of a loan facilitated by the Splendora Cultural Education Facilities Finance Corporation and Frost Bank on behalf of My Possibilities to finance acquisition, construction, furnishing, and equipping of a building located at 3601 Mapleshade Lane, Plano, Texas, Commissioners Court.

2. Acknowledgment of the issuance of a loan facilitated by Wallis Education Facilities Finance Corporation and Frost Bank on behalf of Dallas International School to finance acquiring, renovating, improving constructing and equipping of the Upper School Campus located at 17811 Waterview Parkway, Dallas, Texas, Commissioners Court.

3. Designation of the FY 2019 Commissioners Court regular term meeting dates and time, Commissioners Court.

4. Utilization of Department of Information Resources, IT Staffing contracts for PeopleSoft Development Services through and including September 30, 2019, Information Technology.

5. Payment of an administrative penalty in the amount of $4,501 to TCEQ, Public Works.

6. Restructure of Emergency Management and Fire Marshal departments and FY 2019 budget amendment in the amount of $26,252, Human Resources.

Any action resulting from the executive session.
